What to Bring

When you arrive for your appointments, please make sure to bring the following information:

  • Driver’s License or other photo I.D.
  • Insurance Card(s)
  • List of current medications (i.e. prescription medications and over-the-counter vitamins and supplements)
  • Method of Payment
  • Dental records pertinent to your care (dental records, X-rays, lab results, records from specialist(s))
  • Pre-medicate if needed

Payment Options

For your convenience we accept most credit cards, cash, check, Care Credit, and Lending Club


Insurance

Payments for co-pays and fees for services rendered are due at the time of service. You are responsible for knowing your insurance limitations and policies, as well as understanding what your financial obligation is (deductible, co-insurance, co-pay’s etc.) Please contact your insurance provider to verify we are in network for your plan prior to your visit. Please be sure to bring a current copy of your insurance card to each visit. We cannot submit a claim to your insurance company without specific information listed on the card. We are happy to submit your bill to the insurance company; however, ultimately the payment is your responsibility.

 


Your First Visit

For new patients, we conduct a thorough examination of medical history during the initial appointment. This includes a comprehensive oral examination, a full set of X-rays, panoramic imaging (PAN), intraoral photographs, periodontal probing, and oral cancer screening. While we aim to accommodate a cleaning on the same day if scheduled with a hygienist, the feasibility of this depends on the required type of cleaning.

 


Cancellation Policy

Rollins Family Dental Center has a cancellation policy in place to address no shows and same-day cancellations, aiming to minimize costs while maintaining exceptional care.

Less than 24 Hours' Notice:

  • Hygiene Appointment: $50 fee
  • Doctor Appointment:
    • One-hour appointment: $50 fee
    • One and a half-hour appointment: $75 fee

We understand that unforeseen circumstances may arise, but last-minute cancellations disrupt our schedule and prevent us from accommodating other patients in need of care.


Emergencies

There is a dentist on-call 24 hours a day, 7 days a week to help you with an urgent dental problem. The after-hours answering system is for calls that are urgent and cannot wait for normal business hours. Please dial 847-546-2900 to be directed to the answering service. If your dental condition is life threatening, call 911. If your problem is not a dental emergency, please call during office hours. Prescription refill requests will be handled only during office hours.


Late Arrivals

At our practice, we prioritize the time of our patients. Our aim is to adhere to our schedule to minimize any waiting time for you. If you happen to arrive late for your appointment, we will make every effort to accommodate you on the same day, although there may be a need to wait or reschedule.


Phone Calls, Emails and Text Messages

Our aim is to respond to all messages promptly, yet we kindly request a 24-hour window for returning your call during business hours, unless it's an emergency.

 


Dental Information

All of our staff members are held up to the highest confidentiality standards. Your dental information will remain completely confidential unless we receive a signed written release from you. Information will not be shared with your spouse or family members unless specific authorization is obtained to do so. If you require your records be sent for dental/medical purposes, we can forward records to your physician or other medical facility. This will be done free of charge for continuity of care, however, records copied for patients, insurance companies, or for any other reason, will be charged a fee.

 


Referrals

In the event that a procedure cannot be performed at our office, referrals will be provided onsite. Your referral will be promptly scanned and given to you on the day of your appointment. Subsequently, you will be required to contact and schedule an appointment with the specialist to whom you were referred. Alternatively, if you prefer, we would gladly handle the appointment scheduling process for you.
